Celebrate your child's birthday with their name in Lights - on the front Marquee!
The front marquee will now scroll the names of birthday students for the week of their birthday for $10.00.
(Plus, every student will be recognized on morning announcements on their birthday and get a prize from Mr. Hubbell. You do not need to fill out this form for Principal announcement.)
Does your child have a Summer birthday?
Many parents will celebrate on the half-birthday or will submit a form for the last week of school. Be sure to include the actual birthday on the form, and in Additional Instructions state when you would like the recognition to occur.

- Forms need to be turned in 4 weeks prior to your child's birthday.
- Please fill out one form for each child.
